Requests Students or former students may request that the Registrars Office send their transcripts to a third party. Such a third party might be another college or university, a graduate program, a prospective employer or a scholarship donor. Transcripts may not be released without the written authorization of the student or former student or if the individual requesting the transcript has an outstanding account with the college. To make a request, students on campus may complete a Transcript Request form in the Registrars Office. Students off campus should send a written request to the Registrar and provide the following information. 1. Name and name when attended, if different. 2. Current address, phone number, and student identification number or social security number (for identification only). 3. Dates of attendance. 4. Complete address to which the transcript(s) should be sent. 5. Student signature. Transcript requests may be mailed to Registrar's Office, University Hall, Delaware, OH 43015 or faxed to 740-368-3210. Please note that the Registrar's Office does not fax transcripts to other locations. The fee for each official transcript is $3 -- cash, personal checks, or money orders are acceptable means of payment.For more information, e-mail registra@owu.edu or call 740-368-3200. |
